Case Manager (30 hours per week)
Work where your talent makes a big difference! Bridge of Hope BuxMont works to end homelessness for single mothers and their children in Pennsylvania’s Bucks and Montgomery counties. Your role as a compassionate, enthusiastic Case Manager will empower our clients to become strong, resilient families.
Your Role:
After onboarding and training, you’ll be ready to use your education and experience to provide case management and support services to our participating families. You will report to the Program Manager. Your role includes:
- Promoting family resilience and well-being by providing opportunity for individualized screening and linkage to community resources in the service area of Bucks (Upper and Central) and Montgomery County in PA.
- Engaging in the family intake procedure, and maintaining case records/documentation for each participant
- Coaching and engaging families in goal setting, budgeting and financial education
- Developing Neighborhood of Support and training volunteer groups
- Supporting organization activities and connections to the community
What We Can Offer You:
- $40,000 – $43,000 (salary commensurate with education and experience)

What a Successful Candidate will need:
- Undergraduate or graduate degree in social work, counseling or related field.
- 3 years of case management experience. Preference given to applicants with experience in working with women and children, especially in working with families who are homeless.
- Knowledge and skills implementing a strengths-based trauma-informed approach with a spirit of cultural humility.
- A drive to learn and grow within the social services and non-profit sector.
- Active member of a Christian faith community that confesses Jesus Christ as Lord and Savior.
- Willingness to work occasional evening or weekend hours as needed to meet the needs of the organization and the families we serve.
- A valid driver’s license, automobile insurance, and availability of a vehicle.
- Criminal, Child Abuse, and FBI Background Check Clearances required.
